Just like it was hard to find a website with a table, it was also hard to find a website with a form that has at least six different input types because that is a lot of types. However, my dad is a trucker, so he was able to think of a website that has such a form. Therefore, I will be reviewing the membership form for the Owner-Operator Independent Drivers Association. The six different types of inputs it contains are text boxes, dropdown lists, an email box, a phone box, radio buttons, and a checkbox. The navigation bar on the top of the page just has a whole bunch of different links to different parts of the website, and there is a dropdown menu providing more detailed navigation. On the form page is a picture at the top with what I assume is the company’s motto. Below that is their logo and a little paragraph talking about themselves. Below this is a portion of text talking about membership renewal. Finally, the rest of the page contains the membership form.

The top three sections are small, and they are membership information, additional driver information, and if you wish to enter it, the form lets you enter spouse information. There are only a couple of radio buttons, text boxes, and dropdown lists in these three sections. The rest of the form is personal information and payment information. These sections contain the three input types mentioned above as well as email, phone, and checkbox. Finally, at the bottom, there is the submit button. As for style, there is not much, but the form is laid out well. The form just has the default white background color with nothing to set it apart from the rest of the page. Nothing in the form is styled either. However, like I said, it has a nice layout. Everything is nicely spaced, and it is clearly shown what information goes where. The sections also make it even more organized.

Validation of the form page did give a list of 123 errors and warnings, but there appears to be about an equal amount of each.
